Medical Records

Established patients may request medical records be transferred to a different practice and/or obtain a paper copy for your own records.
​
If you have already established with another practice and need records transferred, you may either fill out and return the following form, or call the office and request the transfer. If you use the paper form, this needs to be faxed, mailed, or dropped off to us in person (cannot be emailed since it is not secure/HIPAA compliant).

To request a transfer of records via phone, during business hours please call 616-971-0060, press option 1 at the first prompt and then option 2 for medical records. If there is no answer, you may leave a message with your full name, date of birth, and the name, address, and fax number (if you have it) of the physician or practice you are transferring to.

If you would like a copy of your medical records/chart for your own records, you may request this via phone or use your patient portal account to download your chart.

If there is no answer, you may leave a message with your full name, date of birth, return phone number, mailing address, and a brief explanation of your request.

To use the AthenaHealth patient portal for downloading your chart, login to your patient portal account. In the menu on the left, click "My Health". Then, in the menu at the top, click "Health Records". Under "1. Choose a time frame", Change the date range of the records you would like to download or select All Time. Under "2. Choose how you'd like to get your data", click the "Download" button.​​​
